Instructions for completing the application
You should complete the application for employment accurately and in its entirety. As you complete the application, please be advised that we check employment references, including information with respect to salary. Please do not make references to your resume or other supporting documentation on the application. All questions require a response. If you are unsure of dates or past compensation, indicate your uncertainty on your application. In addition, if you accept an offer of employment with Joint Commission Resources, you will be asked to sign a consent and release form authorizing Joint Commission Resources to conduct a criminal background check.
You must sign and date the application in three places: - on the last page of the application, under "Business References"
- on the Records/Information Release Form
- on the Employment Application Addendum (Consent and Release) form
