Pediatric Patient Safety Survey

Measuring Pediatric Safety in the Emergency Department

Introduction
Joint Commission Resources developed a survey instrument to measure the pediatric
safety environment in a hospital emergency department. The purpose of this instrument is to
assist emergency departments in identifying opportunities for improvement and to track
improvement in pediatric safety over time.

As part of our effort to create a useful instrument for improvement purposes, we have already recruited a
number of hospitals to complete this survey via this website. Their anonymous responses have been combined to
create a reference (e.g. benchmarks) so that hospital EDs will be able to see how their scores
compare to others.

Now you can view your results against other hospitals like yours!

Survey Procedure
In order to get a complete picture of the ED and hospital environment, we ask that the following
people complete the survey:

-Senior leaders (people who have responsibility for the entire organization, such as a
Chief or Vice President, or equivalent;
-Department Head or manager (people with management responsibility in the ED);
-Physicians, including hospitalists;
-Staff, including nurses, aides, technicians, or allied health staff.

Each hospital should recruit at least 2 or 3 people in each of the four groups, more for front line
staff, if possible. There is no maximum number, you may have as many people take the survey
as you wish. It is best if each person only complete the survey once. Please note: The survey
does not identify individuals and we will not report individual results.

The survey is entirely online and requires about 10 minutes to complete. You may preview a
special version of the survey by using the link below.
