I am wondering what IPs at other facilities log when collecting information on patients with MDRO's?  Do you log all patients with an MDRO or only MDRO HAIs?  Currently our facitlity does not have an IC software program so I enter everything manually. I log every patient with an MDRO (inpatient and outpatient), admission or last visit date, source of admission (home, LTC, etc), culture date, culture site, nursing unit 48 hours before the time of culture, any known recent antibiotic usage, date isolation initiated, Community Associated vs Facility Specific Hospital Assiciated.
It has been beneficial information to have during the risk assessment, but it is very resource intensive and I am wondering if I am collecting more info that other facilities? 

Does CMS or JC require that you keep a log of every patient with an MDRO?

i think the questio was "does TJC or CMS reqire that we keep a log of any patient with aMDRO.  I have the same question. Do you have access to the SIG, or do we need to go to them ourselves with that question?
