A part of the Speak Up initiative, this brochure gives patients guidance on how to find out about their medical condition, new medicines, and follow-up care. An important part of a patient’s recovery is making sure that after leaving the hospital they get the care needed to get better. They are given important directions about follow-up care which, if understood and followed, give them a greater chance of getting well faster and reduce the likelihood of a return visit to the hospital. This brochure provides tips to help patients get more involved in their care. It also gives them the information needed for the best possible recovery.
