The Project RED (Re-Engineered Discharge) intervention is a patient-centered, standardized approach to discharge planning and discharge education, initially developed through research funded by the Agency for Healthcare Research and Quality (AHRQ). AHRQ has now funded JCR to assist hospitals implement the Project RED intervention, aimed at improving the patient's preparedness for self care and at reducing the likelihood of readmission.
Of the 39.5 million hospital discharges per year, 19% of patients have a post-discharge adverse event and 20% of Medicare patients are readmitted within 30 days. Staggering numbers for a process that is not standardized. Learn more about participating in Project RED to implement improvements in your organization that will improve patient safety and save money.
JCR hosted a free web conference that discussed the details of the Project RED intervention, including the eleven components, the role of the discharge advocate and contents of the patient’s After Hospital Care Plan. Project RED intervention can provide value to both the patient and hospital, toward discharging a safer and more educated patient and family, and reducing the likelihood of readmission. Details about how to participate in the AHRQ-funded Project RED implementation activity, managed by JCR, were also provided.
