Ms. Hardy-Leone has more than 25 years of experience in home care and hospice. She is the executive director of a health system–affiliated home care organization. She is responsible for the management and administration of a Medicare-certified home health agency, a Medicare-certified hospice, private services program, and an ADA Certified Diabetic Self-Management Training Program.
Ms. Hardy-Leone’s consulting expertise includes operational assessment, patient safety assessment, medication management, infection control and prevention, environment of care, performance improvement, and incorporating tracer methodology and the periodic performance preview as management tools.
Ms. Hardy-Leone was a home care surveyor for The Joint Commission for more than 13 years. She was a surveyor at the home care program's inception and has surveyed independent, national, and hospital-based home care organizations across the country and served as a preceptor for new surveyors.
