Project RED Success Story
New York City Health and Hospitals Corporation (HHC) successfully engaged its 11 member hospitals in a system-wide implementation of Project RED. Between August 2011 and August 2012, six of 11 hospitals saw an average improvement in CHF readmissions of 5.9%.
HHC’s approach started at the most senior level of the system with a call for each member hospital to identify a senior executive as the project advocate for that hospital. This person recruited a team and, with JCR’s help, led the implementation of Project RED using the Project RED implementation guide.
Among the lessons learned were:
Importance of the pharmacist’s role
Need to engage with the home health agency to learn specific challenges faced by patients
Importance of the social worker’s role
Need for regular cardiology consult
Based on this initial success, HHC is expanding the program to cover additional diagnoses.
