Overview:

This one day, multi-faculty program is designed to provide health care professionals, like you, with strategies and insights to help your organization be successful in achieving Primary Care Medical Home (PCMH) certification.  Certification proves to professionals and the public that your organization is genuinely focused on care coordination, access to care, comprehensive services, and how effectively a primary care clinician and interdisciplinary team work in partnership with the patient (and when applicable, their family). This program will provide you with the answers, tips, tools, and the guidance you need for your organization to become PCMH certified!

Any healthcare related professional interested in Joint Commission’s certification standards for PCMH! The Joint Commission is accredited to provide continuing education credit/hours by the following organizations:

JOINT ACCREDITATION (ACCME, ANCC):
The Joint Commission is accredited by the Accreditation Council for Continuing Medical Education (ACCME) and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

ACHE:
The Joint Commission is authorized to award X hours or pre-approved ACHE Qualified Education credit for this program toward advancement, or recertification, in the American College of Healthcare Executives.  Participants in this program wishing to have the continuing education hours applied toward ACHE Qualified Education credit should indicate their attendance when submitting an application to the American College of Healthcare Executives for advancement or recertification.

CJCP® (Certified Joint Commission Professional®):

To obtain CJCP credit hours, individuals must first be certified before they may start acquiring CJCP credit hours. CJCP credit hours will not be retroactive. CJCP professionals are required to maintain 30 CJCP credit hours within their three-year certification period.

Criteria for Successful Completion:

  • Full attendance at every session is a prerequisite for receiving full continuing education credits
  • Completion of program evaluation form
  • NOTE: For Pharmacists, you will be required to provide your NAPB # and DOB (MM/DD)

Cancellations, Substitutions, and Transfers

Attendee Cancellations– 30 or more days prior to the event:

If you need to cancel your attendance at a live event, your registration fee, less a $100 processing fee, is refundable, if written notice of cancellation is received via e-mail to the below mailbox 30 days or more prior to the program date. Please include the attendee name, program name, date and order #.

Attendee Cancellations - less than 30 days prior to the event:

If you need to cancel your attendance at a live event less than 30 days prior, you may send an alternate attendee.  Please include the registrant’s name, program name, date, and order number as well as the alternate attendee’s full name, title, credentials, address, phone, and email address. Refunds will not be issued for cancellations received less than 30 days prior to the program or any no-shows.

If a program must be cancelled, the registration fee will be refunded in full to each registrant. Substitutions, Transfers, and Cancellation Policy

Attendee Cancellations– 30 or more days prior to the event:
If you need to cancel your attendance at a live event, your registration fee, less a $100 processing fee, is refundable, if written notice of cancellation is received via e-mail to the below mailbox 30 days or more prior to the program date. Please include the attendee name, program name, date and order #.

Attendee Cancellations - less than 30 days prior to the event:

If you need to cancel your attendance at a live event less than 30 days prior, you may send an alternate attendee. Please include the registrant’s name, program name, date, and order number as well as the alternate attendee’s full name, title, credentials, address, phone, and email address. Refunds will not be issued for cancellations received less than 30 days prior to the program or any no-shows.

Event Cancellations

Joint Commission Resources reserves the right to cancel or reschedule a program due to an insufficient number of registrants or other unforeseen circumstances. If a program must be cancelled, the registration fee will be refunded in full to each registrant. You may verify current program status on our website or by calling JCR Customer Service at 877.223.6866. Neither Joint Commission Resources nor The Joint Commission is responsible for a registrant's travel expenses in the event a program is cancelled. Participants are encouraged to purchase refundable tickets in case a program is cancelled or rescheduled.

Dietary Restrictions

Should you have any dietary restrictions for an upcoming event, please contact customer service via e-mail with the name and date of the program and any special accommodations that may be needed. We will work with the catering staff to try to accommodate your needs. In some cases, it may be best to bring some of your own food to supplement what is provided.
